Him hitting on ponies so quickly seems a little odd as I remember House was more than a little weirded out when he had a patient who was very into Cows. Yes the ponies here are basically as intelligent as humans and such, but can't help but feel like he'd be a little off balance with the hitting on ponies maybe after a couple of days or something. I mean he's going to go back ot his world eventually so yeah.

Plus the hospital with basically do the same thing but they would not put you under arrest for being drunk. They would probably put you in the room into you recovered if they had any extra beds.

Easiest, most obvious, and easiest to rule out is if they’re drunk or drugged. The ‘improves with rest’ bit is important. Without that detail, it could be a dozen things off the top of my head, but they’re probably drunk.
Otherwise, I’d toss them in a CT while a CBC,CMP, and electrophoresis is running. Granted my first thoughts like viral meningitis or cerebral swelling don’t fit with the ‘better with rest’ bit.

Twilight's emotional outburst here was unintentional, but it revealed her own prejudices of which she probably wasn't aware.
