Heartbreak, after a roller-coaster ride of an adventure, is finally in a home of her own, and an honorary Cutie Mark Crusader to boot! Unfortunately, her time with the Crusaders and the Summer Readers Club is more than just social calls - it's also a one-of-a-kind opportunity for Equestrian germs.
It's Fluttershy's turn to teach Heartbreak a lesson in friendship, but it will be a struggle to get a real understanding of kindness through to the sickest, most stubborn tan mare in Equestria.

Oh dear, Ghost Writer was channelling Outsiders from Beyond, and because they suck at poetry he doesn't suspect a thing. (No those are good poems actually.)
One thing H.B. has to learn about Pinkie Pie is you always smile at Pinkie Pie, because the longer you don't smile, the more it escalates, and the more it escalates, the harder it is to smile. Pinkie doesn't understand the difference between surface and substance, so instead of working hard to bring you meaningful happiness whether or not it makes you smile, she becomes obsessed instead purely with making the corners of your mouth turn upward, and if that upsets you well, you smiled so it's all good right? She learned a hard lesson whith Discord about how laughter can hurt people when you laugh at them, but she's never learned the lesson that you can hurt people by making them laugh. The people who laugh can be hurting too, not just the ones being laughed at.

HB current condition certainly makes sense... new body no defenses... though why she didn't get sick soon is a good question.
Unless...
I wonder if this Cursed Brand demands HB be social so as to highlight her status as a leper, rather that let her escape into the life of a solitary hermit.
AND...
It does become clear that HB is not a real pony. (The animals, The smells, The brand, The lack of hoof power, the general lack of earth pony vitality...)
She is only a Human Male Twisted into a Pony shape.
All the weaknesses of humans and ponies and none of the benefits...
This most likely WILL end with an Orbital Friendship Cannon to the face... If only to fix her...
Being a horrific pony shaped abomination cannot be good for long term health... and the fae certainly wouldn't REALLY care if she died... as long as every moment she lived was agony. (And considering the implication that her death would be chaotically catastrophic... They bank on even further prolonged suffering in a vain attempt to overcome / fix what the fae deemed an inescapable condition...)
Methinks the hospital is going to wonder why her organs don't look right... And the general stalled plot will finally get its jumpstart...

I'm probably going to do another re-reading of this entire series again; especially since it's been a while since the last update, no rush though.
I've just thought of two interesting things that came to mind and I'm actually curious about what will happen with them unless I forgot some mention of them earlier. The first is Discord, which will probably be mention in this arc, and how he will react to HB. I actually can see him being the one that she learns the friendship lesson from some the bearers never seem to get it.
The second thing that came to mind has to deal with Tirek. What in the world would happen if HB came face to face with him? Would he recognize the magic on her and just leave her alone or would he be an idiot and drain out the unfamiliar magic that is stored in her. If he does drain her what would happen and would the fae magic even return after he was defeated?
